Does these slide on or clamp over the forks? Do the forks need to be removed to install?

It depends on whether or not you need the adapters. If you've got 41mm forks, you'll need to slide them onto the forks. This is due to the silver inner sleeve you see, for 41mm forks, that cannot be separated. If you've got 43mm diameter forks, you can put them on without removing the forks since the two halves can be separated.
