Will this fit a 53 panhead

Yes, if you were to choose, for example, the 1953 FLEF Hydra Glide, you'd receive the correct clutch kit to fit your machine since all Panhead models from that period used the same clutch components.
Does this replace everything except the clutch basket (shell)?

That's correct. Just take everything out of the basket, and put this clutch pack in its place. It'll work with all of your FLH's remaining OEM components.
Is this the clutch basket with the clutch plates already put in place,all I would need to do is take the old one out and replace with this one

Almost everything - it's basically a clutch pack with all of the internals and the outer pressure plate that fits into the OEM clutch basket. Just pull the entire OEM clutch assembly from your machine and put this in its place. It comes with clutch friction plates, steel separator plates and diaphragm spring. There's no need to buy any additional parts to swap out your Fat Boy's clutch assembly.
